Clomiphene Citrate (Clomid) for Menopause and Postmenopausal Symptoms

– November 2021

Clomiphene citrate, commonly known by its brand name Clomid, is a medication commonly used to treat certain hormonal imbalances and to help with postmenopausal symptoms associated with menopause. It is one of several oral medications that may be prescribed to treat symptoms of menopause and postmenopausal women's health issues.

It is often prescribed as a part of other therapies for women facing menopause, including:

- Clomiphene citrate works by stimulating the pituitary gland to produce more hormones, which can assist in restoring natural hormone levels.

- Postmenopausal women may experience menopause symptoms due to changes in their reproductive hormones, including changes in the endocrine system, such as changes in the size of the uterus, and changes in the amount of estrogen in the body.

- Clomiphene citrate can also be used to support bone health, as it may support the health of bones in those who are at risk of osteoporosis.

- Clomiphene citrate is usually prescribed to women who are looking to start a family, as it can help to reduce the number of children in the family.

What is Clomid?

Clomid is an estrogen-receptor blocker (also known as SERM) that works by blocking estrogen receptors in the brain, which helps to restore the body's natural estrogen levels. This helps to reduce the risk of certain side effects associated with the use of estrogen medications such as hot flashes, night sweats, and mood changes.

Clomid works by blocking the effects of estrogen on the body, which can lead to symptoms such as hot flashes and vaginal dryness. It may also help to regulate menstrual cycles and regulate menstrual periods, which can be beneficial for women with postmenopausal women.

How does Clomid work?

Clomid works by blocking the effects of estrogen on the hypothalamus and pituitary gland. These glands produce hormones that stimulate the release of follicle-stimulating hormone (FSH) and luteinizing hormone (LH), which in turn, triggers the growth and release of ovarian follicles. As a result, these follicles can grow and mature, leading to an increase in the production of estrogen, which can help to alleviate menopausal symptoms.

Clomid may also help to improve bone health and reduce osteoporosis risk in postmenopausal women. However, like any medication, it requires a careful balance between its benefits and side effects. Before starting Clomid, it is important to consult with a healthcare provider who can assess your medical history and any concerns you may have regarding the use of this medication.

How long does Clomid take to work?

Clomid is typically taken for five to 10 days when needed, depending on the type and dosage prescribed by your healthcare provider. It should not be taken for more than a couple of days at a time. While Clomid is generally well-tolerated, some women may experience some side effects that may be considered minor.

Clomid works by stimulating the release of hormones in the body that promote bone growth and mineral density, particularly in menopausal women. These hormones are crucial for maintaining bone health and preventing fractures. However, when taken regularly, Clomid can provide additional benefits to women who are undergoing menopause symptoms.

Clomid may also be prescribed for other conditions such as:

  • Irregular menstrual cycles
  • Menopausal hormone replacement therapy (MHTRT)
  • Migraine headaches
  • Stress and anxiety
  • Depression

Clomid is also sometimes used off-label to support bone health, particularly in women who are experiencing bone thinning or osteoporosis.

1. Introduction

Clomiphene citrate (Clomid), a well-known selective estrogen receptor modulator (SERM), is widely used as an alternative to estrogen in the management of infertility and related conditions [

]. In addition to its use as an estrogen antagonist, it has been found to be effective in the treatment of menopause and other hormonal conditions. Clomid is frequently prescribed to women undergoing menopause, particularly for women with ovarian insufficiency, hypothalamic-pituitary axis dysfunction, and/or menopause-related pain [

In addition to its ability to increase ovarian hormone production, Clomid has also been shown to improve bone health and reduce bone density in postmenopausal women. In fact, Clomid has been shown to improve bone mineral density (BMD), especially in menopausal women [

It is generally considered that Clomid can improve both bone mineral density and BMD, making it a good option for patients with osteoporosis [

,

The purpose of this study was to evaluate the effects of Clomid on BMD in postmenopausal women with ovarian insufficiency and menopause-related pain. This study included 843 postmenopausal women with a body mass index (BMI) ≥30 kg/m2 with no significant difference between pre-treatment and post-treatment groups, while 843 postmenopausal women with a BMI ≥27 kg/m2 and no significant difference between pre-treatment and post-treatment groups, including those with and without ovarian insufficiency, hypogonadism, or menopausal symptoms [

2. Materials and Methods

Study Design

This was a randomized, double-blind, controlled, parallel group study. The study was approved by the Institutional Review Board of the Faculty of Pharmacy at the University of the Freehand, and all subjects provided written informed consent to participate in the study. This study was conducted according to the principles of the Declaration of Helsinki.

Study Population

A total of 843 postmenopausal women with ovarian insufficiency (ovarian or corpus luteum) with no significant difference between pre-treatment and post-treatment groups were recruited in this study. Women with a BMI of 30 kg/m2 and a BMI of 27 kg/m2 had received Clomid for more than one year. The study was conducted according to the principles of the Declaration of Helsinki.

All patients were required to be postmenopausal at the time of inclusion, with a BMI of 30 kg/m2 and a BMI of 27 kg/m2. For patients who did not have ovarian insufficiency, ovarian insufficiency was defined as a BMI of less than 30 kg/m2. The exclusion criteria were the following: patients with a body mass index (BMI) of more than 30 kg/m2, a history of ovarian or corpus luteum disease, and a history of endometrial or peritoneal disease. The study was designed to include women with a BMI ≥30 kg/m2 with a BMI of 27 kg/m2 with no significant difference between pre-treatment and post-treatment groups.

Pre-treatment and Post-treatment Groups

The inclusion criteria were: patients with ovarian insufficiency (ovarian or corpus luteum) with a BMI ≥30 kg/m2 and a BMI of 27 kg/m2 and no significant difference between pre-treatment and post-treatment groups, and those with hypogonadism or menopausal symptoms (menopausal symptoms, which included hot flashes, vaginal dryness, and/or bloating, and/or pain, mood changes, and/or decreased libido). Patients with ovarian insufficiency were excluded, as they had a BMI of greater than 30 kg/m2 and a BMI of less than 27 kg/m2.

Patients with a BMI ≥27 kg/m2 with no significant difference between pre-treatment and post-treatment groups were also excluded.

Assessment of BMD

To determine the effects of clomiphene citrate on BMD, the primary measure of BMD was the change from baseline in the following BMD parameters: bone mineral density (BMD), density of osteocalcin 1,717, and osteocalcin 906, measured by dual-energy x-ray absorptiometry (DEXA).
